Discal Cysts (Intraspinal Cyst Communicating With the Intervertebral Disc): Sixteen Cases and Review of the Literature

Abstract
Several types of intraspinal cyst with different pathogenesis have been reported in the past, such as perineural cysts, synovial cysts, arachnoid cysts, and ganglion cysts. Chiba et al was reported 8 cases of distinct intraspinal cysts communicating with the corresponding intervetebral discs, and proposed this entity be called a discal cyst. Discal cyst manifests symptoms and signs resembling those of intervertebral disc herniation. We reported sixteen cases of discal cysts with histopathologic and radiologic findings. The cyst wall consists of dense fibrous connective tissue containing bloody and clear serous fluid without specific lining epithelium. Absence of neovascularization, cellular infiltration or disc materials are also typical findings. Histologically It is difficult to differentiate from ganglion cysts, but either discography/CT discograpgy or MRI reveals characteristic findings: it shows the connection between the discal cyst and the disc. We reviewed 16 cases of this distinct entity and literature.
